Students are required to complete and submit a permission slip, student info form, behavior contracts, and travel group release. Click here to download these forms and bring them to the January 13th booster meeting!

Instructions for a My Disney Experience Account - if you register for fast passes, please view the tentative itinerary to make sure that you are getting fast passes for the parks we will be in each day. Note that everyone must have a buddy at all times... so be sure your buddy is also getting the same fast passes with you! To get fast passes together students will need to link their Disney My Experience accounts together, that is the only way the fast passes will in all probability be at the same time. This may take some planning and communication prior to registering for fast passes. Also note, that if the itinerary changes, you may not be able to use all of your fast passes (there is no penalty, they will just expire).
Prescription Medication Form (Parent Request & Physicians Order): Required for any prescription medications taken by students during the trip. Details from school nurse regarding school policy on Prescription and Over the Counter Medications. Forms must be returned to no later than March 8th for preparation by the nurse.

Bus Release: All students MUST ride the bus to Disney but do not have to ride the bus back to Apex. If not riding the bus back please complete this BUS RELEASE FORM and communicate with your student's chaperone.

APEX HIGH BAND DISNEY WORLD TRIP 2015

Who:Any student who participates in any Apex High Band
General plan for the Disney trip, including cost, dates and payment coupons.
Please note that the first payment of $200 is due by October 25th.
Those payments may be given to Indira Valencia after practices, placed in the envelope in the band room, or sent to the following address:

There are payment coupons attached to the document above. Please use those with your payment and include your student's name on the check. Note on the coupon if you would like the payment to be deducted from their student account. Contact Dick Warner (treasurer) at firstname.lastname@example.org if you need to know your account balance. Also, if you would like to be a chaperone your first payment is due on the same day. Please note that you would like to be a chaperone and be sure that you have registered as a volunteer with Wake County.
